Modules over Polynomial Rings

Let $k$ be a field and $V$ be a vector space over $k$. Then, the $k[x]$ action on $V$ is given by a linear transformation $T$ that sends a polynomial $p(x)\in k[x]$ to the matrix $p(T)$.
